29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about 1/4/18, the employer failed to ensure that machine guarding methods were in place. Machinery includes, but is not limited to the following:    a) The Bright Anneal line, had a pinch point between the stationary table and the gaping block that opened about 4" during operation, and    b) The welder carriage of the Bright Anneal line created a pinch point between the electrical box and the Welder Operator panel when the carriage slides to the out position during welding operations, and     c) The welder carriage of the Bright Anneal Line moved in and out creating a struck-by hazard.
