29 CFR 1910.147(c)(4)(ii):   Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  The employer's written Lockout/tagout procedures did not address the method to be used to control hazardous energy such as but not limited to Hydraulic and pneumatic potential energy sources that could result in serious injury.  This was documented on  January 10, 2018 at 213 Ave A, Bayard, Nebraska.  The procedure provided by the employer for the Cement mixer identified three energy sources, electrical, hydraulic, and pneumatic.  Although three sources were identified on the procedure, only the electrical energy source was listed as being locked or tagged out.

29 CFR 1910.147(c)(6)(ii):   The employer did not certify that periodic inspections of the energy control procedures had been performed:  The employer failed to ensure that  a certification of the periodic review of the specific written energy control procedures was produced.  This was most recently documented on January 10, 2018 at 213 Ave A Bayard, Nebraska.  The employer provided lockout/tagout procedures but could not produce documentation to indicate that the written procedures had been reviewed.  The written procedure provided was developed in 2004.

29 CFR 1910.242(b):   Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.:  The employer failed to protect employees from flying debris and air embolism hazards.  This was most recently documented on January 10, 2018 at 213 Ave A. Bayard, Nebraska.  The employer failed to ensure that compressed air used for cleaning the debris that was created in the production of burial vaults was reduced to 30 p.s.i. The air pressure present at the time of the inspection and used by employees that morning for cleaning purposes was 95 p.s.i.
