29 CFR 1926.1060(a)(1)(iii): The employer did not ensure that each employee using ladders and/or stairways had been trained by a competent person in the proper construction, use, placement, and care in handling the ladders and/or stairways:    Extrusion R&D Area- An employee working from a Werner 8 foot fiberglass stepladder was not provided training in the safe usage and placement of stepladders by the employer. On or about December 26, 2017 an employee working from an 8 foot stepladder did not have the ladder placed in the direct vicinity of all work areas and over reached beyond their immediate work area. The employee lost balance while over reaching and tipped the stepladder over causing the ladder and employee to fall to the ground.
