29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): (a) Facility, 12731 Route 30 W, Irwin, PA: On or about January 16, 2018 the socket on one of the lighting systems located at dock 13 had been pulled from the connection and exposed the wiring. The power was on to the lighting system and exposed employees to potential electrical shock hazard when moving the lamps to illuminate the work area.

29 CFR 1910.28(b)(1)(iii)(C): When the employer could demonstrate that the use of fall protection systems is not feasible on the working side of a platform used at loading rack, loading dock or teeming platform the work was done without fall protection systems by authorized employees who are not trained in accordance with �1910.30: (a) Facility, 12731 Route 30 West, Irwin, PA: On or about January 16, 2018 the employer did not provide fall protection systems at the loading docks nor were employees who work in the area trained in accordance with 1910.30 to ensure they understood the nature of fall hazards in the work area and how to recognize them and what procedures are to be followed to minimize those hazards.
