29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a) Rear of Right Side of Structure, Third Floor Level - Two workers were in the basket of a telescopic boom lift extended approximately 30 feet high, finishing the installation of a sliding glass door and neither worker was wearing fall protection on or about 1/10/2018. No abatement certification or documentation required for this item.

29 CFR 1926.503(b)(1): The employer did not verify fall protection training by preparing a written certification record.(a) a) Rear of Right Side of Structure, Third Floor Level - The employer failed to provide certification of fall protection training where employees were not protected against falls while exposed to a fall hazard of approximately 30 feet on or about 1/24/2018. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
