On 11/14/2017, employees of AZ Contracting LLC were working on the roof of a residential building located at 106 Crocket Creek Drive, Wentzville, MO. Eye protection was not used by employees involved with using pneumatic nail guns during roofing operations.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system. On 11/14/2017, employees were working on a residential building located at 106 Crocket Creek Drive, Wentzville MO. Employees were installing and removing roofing materials approximately 10 feet above a lower level. Employees were not utilizing guardrail systems, a safety net system, or a personal fall arrest system. The employees were observed on the roof of the building without fall protection, exposing themselves to a fall to the ground below.
