29 CFR 1926.451(b)(3): The front edge of all platforms was not 14 inches (36 cm) or less from the face of the work, and no guardrail systems were erected along the front edge and/or no personal fall arrest systems were used in accordance with paragraph (g) of this section to protect employees from falling.    462 Boca Raton Dr., Toms River, NJ: On or about January 16, 2018, two 3WK Contractor LLC employees working from an aluminum pump jack scaffold were the working platform was greater than 14 inches from the face of the work.  The employees were not using any form of fall protection, thus exposing them to fall hazards of greater than 20 feet to the ground below.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  462 Boca Raton Dr., Toms River, NJ: On or about January 16, 2018, two 3WK Contractor LLC employees were working from an aluminum pump jack scaffold installing weather shield.  The scaffold did not have mid rails or end stops and the employees were not using any other form of fall protection, thus exposing them to fall hazards of greater than 20 feet to the ground below.

The support poles were not secured in any manner, exposing the workers to potential fall hazards of greater than 20 feet to the ground below.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:     462 Boca Raton Dr., Toms River, NJ: On or about January 16, 2018, a 3WK Contractor LLC employee was installing siding weather shield from a portable extension ladder.  The employee climbed onto the roof to continue securing the shield.  .  The employee was not using any form of fall protection and was thus exposed to fall hazards of greater than 20 feet to the ground below.
