29 CFR 1910.111(b)(9)(ix): A hydrostatic relief valve was not installed between each pair of valves in the liquid ammonia piping or hose where liquid could be trapped so as to relieve into the atmosphere at a safe location: At the time of the inspection, each hydrostatic relief valve was installed with a ball valve before the relief valve which would not allow the trapped liquid to release if the ball valve was closed. Industry practice does not allow the placement of valves inline before a pressure relief device. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citations.

29 CFR 1910.119(e)(3)(iv): The process hazard analysis did not address the consequences of failure of engineering and administrative controls: At the time of the inspection, the most recent PHA from 11/20/2014 did not address the consequences of failure of pressure relief devices that had ball valves installed inline in front of the relief devices. The PHA did not address the administrative control of requiring employees to keep the valves in the open position to prevent the failure of piping if the relief device was blocked. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. 29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical. At the time of the inspection, Anhydrous Ammonia storage tanks were not labeled with the hazards of the chemical.
