29 CFR 1926.651(h)(1): Employees were working in excavations in which there was accumulated water, or excavations in which water was accumulating, and adequate precautions had not been taken to protect employees against the hazards posed by water accumulation: a. At the intersection of US Hwy 301 and County Road 525 in Sumterville Florida, employees were exposed to a caught in hazard, in that, during non-destructive testing of steel piping in a trench with type "C" soil which was 9 feet deep, 22 feet long and 9 feet wide, water was allowed to accumulate while an employee was conducting non-destructive testing, on or about 01/17/2018.

At the intersection of US Hwy 301 and County Road 525 in Sumterville Florida, on or aboutr 01/17/2018, a competent person did not conduct inspections of an unprotected trench with type "C" soil which was 9 feet deep, 22 feet long and 9 feet wide prior to an employee entering the trench and as needed throughout the shift while an employee was working in the trench.

At the intersection of US Hwy 301 and County Road 525 in Sumterville Florida, employees were exposed to a 6.5 foot fall hazard, in that a ladder used to gain access in and out of the trench at the west wall did not extent three feet above the landing surface, on or about 01/17/2018.
