The employer must ensure: (i) Each employee in a hoist area is protected from falling 4 feet (1.2 m) or more to a lower level by a guardrail system, a personal fall arrest system, or a travel restraint system: On or about January 17, 2018, employees on lines 1-6 were exposed to fall hazards of over 5 feet as the hoist areas were protected with a single chain that was measured at 25 inches in height.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. On or about January 17, 2018, employees were exposed to ingoing nip points on unguarded conveyors feeding the mixers on lines 1-6.
