29 CFR 1926.651(h)(1): Employees were working in excavations in which there was accumulated water, or excavations in which water was accumulating, and adequate precautions had not been taken to protect employees against the hazards posed by water accumulation:  a. At the intersection of US Hwy 301 and County Road 525 in Sumterville Florida, employees were exposed to a caught in hazard, in that, during non-destructive testing of steel piping in a trench with type "C" soil which was 9 feet deep, 22 feet long and 9 feet wide, water was allowed to accumulate while an employee of a subcontractor was conducting non-destructive testing. As a controlling employer the Raw Construction competent person did not remove the employee of the subcontractor from the unsafe trench conditions, on or about 01/17/2018.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. At the intersection of US Hwy 301 and County Road 525 in Sumterville Florida, employees were exposed to a 6.5 foot fall hazard, in that a ladder used to gain access in and out of the trench at the west wall did not extent three feet above the landing surface, on or about 01/17/2018.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).     a. At the intersection of US Hwy 301 and County Road 525 in Sumterville Florida, employees were exposed to a caught in hazard, in that, shoring or sloping was not used to protect employees during the installation of an 8 inch steel piping in a trench with type "C" soil which was 9 feet deep, 22 feet long and 9 feet wide, on or about 01/17/2018.    R.A.W. Construction, LLC was previously cited for a violation of this Occupational Safety and Health Standard 1926.652(a)(1) which was contained in OSHA inspection number 1256440 citation number 1, item number 4 and was affirmed as a final order on 01/09/2018, with respect to a workplace located at Old Broadnax Mill Rd, Loganville, GA 30052.
