29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: (a) New commercial warehouse construction, Front side: On or about 01/16/2018, and at times prior, the employer exposed employees to fall hazards of approximately 24-feet, in that steel pins were not placed at the heels of the forks to prevent the work platform from slipping off during use while occupied by employees performing steel decking activities.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: (a) New commercial warehouse construction, Front side: On or about 01/16/2018, and at times prior, the employer exposed employees to fall/ejection hazards of approximately 24-feet to the lower level, in that a harness with lanyard was not provided to employees working in a safety work platform resting on the forks of a JLG G6-42A Forklift Truck (b) New commercial warehouse construction, Rear side: On or about 01/16/2018, and at times prior, the employer exposed employees to fall/ejection hazards of approximately 21-feet to the lower level, in that a harness with lanyard was not donned prior to operating the aerial lift to gain access to the roof and ground levels.

29 CFR 1926.760(b)(3): Each connector working at heights over 15 and up to 30 feet above a lower level, was not provided with a personal fall arrest system, positioning device system or fall restraint system and wear the equipment necessary to be able to be tied off; or was not provided with other means of protection from fall hazards in accordance with paragraph (a)(1) of this section: (a) New commercial warehouse construction, Roof: On or about 01/16/2018, and at times prior, the employer exposed employees to fall hazards of 24-feet, 7-inches to 27-feet, 2-inches, in that a complete fall protection system was not provided to employees installing metal decking on 1/2 on 12 pitched roof. Employees were not provided with the ability to tie-off or provided with a controlled decking zone.
