29 CFR 1926.502(d)(8): Horizontal lifelines were not designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintained a safety factor of at least two:  (a) On or about January 16, 2018, and at times prior, the employer did not ensure that horizontal lifelines were designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintained a safety factor of at least two. Employees were exposed to falls of up to 30 feet while conducting concrete removal activities using a fall arrest system that included a horizontal lifeline which was not designed, installed, or used under the supervision of a qualified person. 29 CFR 1926.502(d)(11): Lifelines were not protected against being cut or abraded:  (a) On or about January 16, 2018, and at times prior, the employer did not ensure that horizontal lifelines were protected against being cut or abraded. Employees were exposed to falls of up to 30 feet while conducting concrete removal activities using a fall arrest system that included a horizontal lifeline secured to a staircase metal structure with edges that did not offer protection against cuts or abrasion. 29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  (a) On or about January 16, 2018, and at times prior, the employer did not ensure that personal fall arrest systems, when stopping a fall, were rigged such that an employee could not contact any lower level. Employees were exposed to falls of up to 8.5 feet while conducting concrete removal activities using a fall arrest system that included a 6-foot fall-arresting lanyard tied off to a horizontal lifeline which would have not prevented a worker from contacting the ground level in case of a fall.
