Employees were exposed to falls of up to 30 feet while conducting concrete removal activities using a fall arrest system that included a horizontal lifeline which was not designed, installed, or used under the supervision of a qualified person. Employees were exposed to falls of up to 8.5 feet while conducting concrete removal activities using a fall arrest system that included a 6-foot fall-arresting lanyard tied off to a horizontal lifeline which would have not prevented a worker from contacting the ground level in case of a fall.
