29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA: (a) U.S. Postal Service - 43 Star Road, Hereford, Pennsylvania - An employee suffered a left hip fracture, resulting in hospitalization, on January 8, 2018 as a result of slipping and falling on an ice. The employer failed to report this work-related hospitalization to OSHA within 24 hours. (The employer reported this hospitalization to OSHA on January 11, 2018.) The U.S. Postal Service was previously cited for violations of this Occupational Safety and Health Standard 29 CFR 1904.39(a)(2) which were contained in: OSHA inspection number 1027982, citation number 1, item number 1 and was affirmed as a final order on March 12, 2015, with respect to a workplace located at 6330 Fountain Square Drive, Citrus Heights, CA 95621; and OSHA inspection number 1050798, citation number 1, item number 1 and was affirmed as a final order on July 7, 2015, with respect to a workplace located at 632 Vailview Drive, Nashville, TN 37207; and OSHA inspection number 1097986, citation number 1, item number 1 and was affirmed as a final order on November 16, 2015, with respect to a workplace located at 2905 Mars Street, Virginia Beach, VA 23452; and OSHA inspection number 1114054, citation number 1, item number 1 and was affirmed as a final order on February 24, 2016, with respect to a workplace located at 335 Section Line Road, Hot Springs, AR 71901; and OSHA inspection number 1142554, citation number 1, item number 1 and was affirmed as a final order on May 20, 2016, with respect to a workplace located at 87-67 116th Street, Richmond Hills, NY 11418; and OSHA inspection number 1182726, citation number 1, item number 1 and was affirmed as a final order on March 21, 2017, with respect to a workplace located at 1800 James Jackson Parkway, Atlanta, GA 30369.
