29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:      On or about 1/17/2018, at the Production warehouse of the above addressed worksite, the employer did not establish an energy control program, resulting in the exposure of employees to a caught by hazard while performing servicing activities such as, but not limited to;  changing belts of a Heidelberg Stahlfolder; rollers cleaning and blowing air pumps of a MBO folder; rollers cleaning and replacement of an UV Machine, etc.

The employer had not developed and implemented a written hazard communication program for protecting employees from a chemical hazard presented by products  (i.e. Quickset Pantene Reflex, Mega Fountain Concentrate, PR010 Cylinder Cleaner, Nu-flu Thermographic powder, Inks, etc.) which are used for printing purposes on a daily basis.

29 CFR 1910.1200(h)(2): The employer did not provide employees with effective information on hazardous chemicals in their work area at the time of their initial assignment and whenever a new chemical hazard the employees have not previously been trained about was introduced into their work area. Employees were not informed of the following: the requirements of this section; any operations in their work area where hazardous chemicals are present; and, the location and availability of the written hazard communication program, including the required list(s) of hazardous chemicals, and safety data sheets as required by this section.   On or about 1/17/2018, at the Production warehouse of the above addressed worksite.  Employees were exposed to a chemical hazard while using products for printing purposes on a daily basis (i.e. Quickset Pantene Reflex, Mega Fountain Concentrate, PR010 Cylinder Cleaner, Nu-flu Thermographic powder, Inks, etc.) when lacked information, as evidenced by, among other conditions; employees lacked knowledge of location and availability of the company's hazard communication program; employees lacked knowledge of the purpose, location and availability of the required  safety data sheets, etc.
