29 CFR 1910.22(a)(1): (1) The employer must ensure all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition. On or about January 17, 2018 at 3181 Commodity Lane in Green Bay, employer did not remove tripping hazards of cords and hoses which were lying on the walking/working surface for FD Pouch Machine number twelve (12).

Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment. On or about January 17, 2018 at 3181 Commodity Lane in Green Bay, motor control panels and/or E-stop controls were blocked from use on FD Pouch Machine number eight (8), BH Pouch Machine number nine (9), and Folder Machine number fourteen (14).
