29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) West side of building, during roofing work: On or about 1/19/18, two employees were exposed to a fall hazard of approximately 15 to 20 feet while repairing flashings in the roof valley without use of fall protection; roof surface was snow and ice covered.

(a) West side of building, during roofing work: On or about 1/18/18, a fiberglass extension ladder used to access the roof was used at an angle such that the horizontal distance from the the top support to the foot of the ladder exceeded one-quarter the working length of the ladder.
