29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about January 18, 2018, in the glue department, the rollers were not guarded on the Black Brothers glue spreader. Employees add glue and run boards through the rollers on the glue machine. Employees exposed to the hazard of being caught-in the rotating rollers when processing boards.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed: a) On or about January 18, 2018, in the glue department, the chain and sprocket that rotates the rollers were not guarded on the Black Brothers glue spreader. Employees add glue and run boards through the rollers on the glue machine. The employees were exposed to the hazard of being caught-in the rotating chain and sprocket. b) On or about January 18, 2018, in the glue department, the chain on the Black Brothers panel express machine was not guarded. This is a machine that presses the boards after going through the glue machine. The employees were exposed to the hazard of being caught-in the rotating chain.
