29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2m) or more above a lower level was protected from falling by guardrail systems, safety net system, or personal fall arrest system.  On or about 18 January 2018, and at times prior thereto, employees accessed the top of plastic injection molds without fall protection, exposing them to the hazard of falling approximately 8 feet to the floor of the machine below.

29 CFR 1910.176(a): Aisle(s) and passageway(s) were not kept clear and in good repair with no obstruction across or in aisles that could create a hazard.  On or about 18 January 2018, and at times prior thereto, employees walking on the production floor within marked pedestrian walkways were exposed to being struck by moving powered industrial trucks adjacent to the same walkways that were transporting materials on pallets or racks. 1) Press #32: Metal racks and plastic pallets were positioned in a manner that constricted the movement of forklift traffic in and around the adjacent pedestrian walkway, with a clearance of approximately 10 inches to 2 feet. 2) Press #4: While part 8115 is running on the press, the amount of product generated congests the aisle ways of both the forklift and pedestrians, resulting in the forklift operator backing in and out of the pedestrian walkway and near workstations.
