29 CFR 1910.178(q)(7): Industrial trucks were not examined at least daily before being placed in service, and when used on a round-the-clock basis, they were not examined after each shift. a) Second Floor- On or about January 16, 2018, an automated maintenance monitoring system or Pivms terminal relied upon to examine Ox #80 powered industrial truck used to pull wheeled carts of mail was missing. b) Second Floor- On or about January 16, 2018, an automated maintenance monitoring system or Pivms terminal relied upon to examine Ox #32 powered industrial truck used to pull wheeled carts of mail was missing. c) Second Floor- On or about January 16, 2018, an automated maintenance monitoring system or Pivms terminal relied upon to examine Ox #25 powered industrial truck used to pull wheeled carts of mail was damaged and inoperable. d) Second Floor- On or about January 16, 2018, an automated maintenance monitoring system or Pivms terminal relied upon to examine Ox #25 powered industrial truck used to pull wheeled carts of mail was damaged and inoperable. e) Second Floor, On or about January 16, 2018, an automated maintenance monitoring system or Pivms terminal relied upon to examine ,Ox #23, a powered industrial truck used to pull wheeled carts of mail, did not indicate a damaged battery cable. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1910.303(b)(1)(iv): Electric equipment was not free from recognized hazards that are likely to cause death or serious physical harm to employees such as damaged insulation: a) Second Floor - On or about January 16, 2018, the shielding insulation surrounding and protecting the inner copper conductors of a PIT battery cable was damaged thereby exposing workers to 12 volt live parts. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
