29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:      (a) On or about January 17, 2018, in the production area, Machine 101 - OKK CNC Vertical Machining Center (model no. MCV-550, serial no. MA49188) lacked adequate guarding which exposed employees to entanglement hazards.  (b) On or about January 17, 2018, in the production area, Machine 102 - OKK CNC Vertical Machining Center (model no. PCB-55, serial no. 103) lacked adequate guarding which exposed employees to entanglement hazards.  (c) On or about January 17, 2018, in the production area, Machine 103 - OKK CNC Vertical Machining Center (model no. MCV-560, serial no. 150) lacked adequate guarding which exposed employees to entanglement hazards.  (d) On or about January 17, 2018, in the production area, Machine 104 - OKK Vertical Machining Center (model no MCV-460, serial no. 305) lacked adequate guarding which exposed employees to entanglement hazards.  (e) On or about January 17, 2018, in the production area, Machine 109 - OKK Vertical Machining Center (model no. VM-7II, serial no. 410) lacked adequate guarding which exposed employees to entanglement hazards.  (f) On or about January 17, 2018, in the production area, the safety interlocks were bypassed on Machine   302 - DMG Mori Seiki CNC Turning Center Lathe (model no. SL-25, serial no. 6139) which exposed employees to entanglement hazards.  (g) On or about January 17, 2018, in the production area, Machine 303 - DMG Mori Seiki CNC Turning Center Lathe (model no. SL-25, serial no. 1051) lacked adequate guarding which exposed employees to entanglement hazards.    (h) On or about January 17, 2018, in the production area, Machine 306 - Fortune CNC Lathe (model no. Vturn 46, serial no. MB-1256) lacked adequate guarding which exposed employees to entanglement hazards.       (i) On or about January 17, 2018, in the production area, Machine 309 - DMG Mori Seiki CNC Turning Center Lathe (model SL-400, serial no. 53) lacked adequate guarding which exposed employees to entanglement hazards.  (j) On or about January 17, 2018, in the production area, the chuck guard was missing on Machine 404, Howa Lathe (model no. 1500, serial no. 12092) which exposed employees to entanglement hazards.

(a) The employer did not ensure that employees had been properly trained on Hazard Communication, in that employees had limited knowledge of what Hazard Communication was or the hazards associated with chemicals that they use. Employees used chemicals such as, but not limited to: TRIM MicroSol 685 (machine coolant).
