29 CFR 1910.178(a)(6): The employer did not ensure that all powered industrial truck (PIT) nameplates and markings were in a legible condition: 5 Cudworth Rd, Webster, MA On January 19, 2018 and at times prior, employees operated a Combilift C6000ST with a permanently installed manufacturer-approved glass handling attachment with an illegible maximum capacity chart. The chart was the only location on the PIT to reflect that the attachment reduced the maximum capacity from 6,000lbs to as low as 2,920lbs depending on the height of the raised load.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck (PIT) operator was competent to operate a PIT safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): 5 Cudworth Rd, Webster, MA On January 19, 2018 and for a few months prior, the employer had not ensured that all operator employees were competent to operate PIT. Some operators had no documents to back up their training and evaluations from a previous owner and other operators had not been fully trained and evaluated.
