29 CFR 1926.20(b)(1): A safety and health program was not initiated and maintained to provide compliance with the general safety and health provisions of the standard:    On or about 10 January 2018, at the above jobsite, the employer failed to ensure employees operated a circular saw in accordance with the operators manual, failure to operate machinery in accordance with the designed settings can lead to loss of control of the machine, exposing employees to the hazard of amputation or lacerations.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    On or about 10 January, 2018, at the above addressed job site, three employees were working from the roof of the commercial building under renovation, without the employer providing fall protection, exposing the employees to a fall hazard of up to approximately 12 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:     On or about 10 January 2018, on the above addressed jobsite, three employees were accessing the roof of a building with an extension ladder that did not extend at least 3 feet above the landing, exposing the employees to a fall hazard of approximately 12 feet.
