29 CFR 1910.134(f)(2): The employer did not ensure that employees using tight fitting face piece respirators pass an appropriate qualitative or quantitative fit test prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter. (a) On and before 1/17/2018, throughout 1228 Western Ave building, for the demo crew required to wear 3M N95 respirator while doing demo on drywall and floor tiles using sledgehammers, crowbars, and sawzalls. The employee was not fit tested, either qualitatively or quantitatively, prior to being required to use this respirator by the employer.

The employer did not provide information on at least the following hazards: Cancer and lung effects. a) On and before 1/17/2018, throughout 1228 Western Ave building, demolition crew was using sledgehammers, crowbars, and sawzalls in areas where there was asbestos containing floor tiles. The employer did not train employees on the methods of recognizing asbestos and the health effects associated with it.
