29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure. In shop 1, an extension cord connected to a power cord, wrapped around an air condition material and bracket, was used as permanent wiring to energize the Grizzly band saw exposing employees to electrical shock hazards.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws. In shop 1, employer did not ensure strain relief devise was provided to prevent tension on the floor fan power cord exposing employees to shock hazards.
