29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace: Location: 824 Lone Star Drive, O'Fallon, MO The employer did not provide a medical evaluation to determine an employee's ability to use an elastomeric respirator before the worker was required to use a 3M half-facepiece respirator with a P100 No. 2091 filter, in the workplace to prevent exposure to dust. Employees are required to wear respiratory protection during solid chemical adsorbent production tasks to include, but not limited to, operating material handling equipment, mixing dry product, and packaging operations.

29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator, whenever a different respiratory facepiece is used, and at least annually thereafter: Location: 824 Lone Star Drive, O'Fallon, MO The employer did not provide medical fit tests in accordance with the standard to employees required to use a 3M half-facepiece respirators with a P100 No. 2091 filters, in the workplace to prevent exposure to dust: a) One worker was not fit tested prior to initial use; b) Three workers were not annually fit tested.
