29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level: a) Jobsite: The employer did not ensure that employees working from scaffold platform approximately 30 feet from the ground below were protected by a fall protection system. Employees are working from an Ingersoll Rand VR-1044C fork truck which has plywood laid across the forks of the truck to perform window installation operations. This condition noted on/about 15 JAN 2018.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: a) Jobsite: The employer did not ensure that employees working from the scaffold platform to install windows to the exterior of the building were trained in the hazards of the scaffold utilized. Employees were using an Ingersoll Rand VR-1044C with plywood boards laid across the forks to perform window installation operations approximately 30 feet from the ground below without having received training in the hazards associated with the type of scaffold being used. This condition noted on/about 15 JAN 2018.
