29 CFR 1910.134(c)(1): In any workplace where respirators are necessary to protect employee health or whenever respirators are required by the employer, a written respiratory protection program that included the provisions in 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented:  a) Spray Booth: On or about January 17, 2018 and at times prior, employees were exposed to respiratory hazards from chemicals such as, but not limited to; xyol, acetone, ethyl benzene, methoxy propanol and butyl acetate while spraying acrylic lacquer and using lacquer thinner in various phases of the wood treatment process. The employer did not provide medical evaluations to employees required to use the respirators as required in 1910.134(c)(1)(ii).  b) Spray Booth: On or about January 17, 2018 and at times prior, employees were exposed to respiratory hazards from chemicals such as, but not limited to; xyol, acetone, ethyl benzene, methoxy propanol and butyl acetate while spraying acrylic lacquer and using lacquer thinner in various phases of the wood treatment process. The employer did not establish work-site specific procedures to regularly evaluate the effectiveness of the program as required in 1910.134(c)(1)(ix).  c) Spray Booth: On or about January 17, 2018 and at times prior, employees were exposed to respiratory hazards from chemicals such as, but not limited to; xyol, acetone, ethyl benzene, methoxy propanol and butyl acetate while spraying acrylic lacquer and using lacquer thinner in various phases of the wood treatment process. The employer did not provide procedures for cleaning, disinfecting, storing, inspecting, repairing, discarding, and otherwise maintaining respirators of employees required to use the respirators as required in 1910.134(c)(1)(v).  d) Spray Booth: On or about January 17, 2018 and at times prior, employees were exposed to respiratory hazards from chemicals such as, but not limited to; xyol, acetone, ethyl benzene, methoxy propanol and butyl acetate while spraying acrylic lacquer and using lacquer thinner in various phases of the wood treatment process. The employer did not provide training on putting on, removing, or the limitations of the respirators for employees required to use the respirators as required in 1910.134(c)(1)(viii).  e) Spray Booth: On or about January 17, 2018 and at times prior, employees were exposed to respiratory hazards from chemicals such as, but not limited to; xyol, acetone, ethyl benzene, methoxy propanol and butyl acetate while spraying acrylic lacquer and using lacquer thinner in various phases of the wood treatment process. The employer did not provide fit testing procedures for tight-fitting respirators to employees required to use the respirators as required in 1910.134(c)(1)(iii).
