29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) In the coil staging area - On or about January 11, 2018, an employee was operating a Yale powered industrial truck, Model No. GDP100VXNTGE096, S/N No. K813V1801N, equipped with a carriage mounted coil ram, that measured approximately 69 inches in length, without the manufacturer's prior written approval. The powered industrial truck was rated by the manufacturer for use with 48 inch forks on a 54 inch carriage. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: a) In the coil staging area - On or about January 11, 2018, an employee was operating a Yale powered industrial truck, Model No. GDP100VXNTGE096, S/N No. K813V1801N, equipped with a carriage mounted coil ram, that measured approximately 69 inches in length. The truck's identification and load rating plate indicated that the truck was rated by the manufacturer for use with 48 inch forks on a 54 inch carriage. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
