29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.    For the employees working from the welded fabricated frame scaffold on or about January 19, 2018 and times prior there to, the employer did not fully deck the scaffolding at the locations employees were required to scuff the wall surface in preparation of installing the stucco.  This exposes employees to falls from heights greater than 10 feet.

29 CFR 1926.451(b)(7): On scaffolds where platforms are overlapped to create a long platform, the overlap did not occur only over supports:    For the employees working on the scaffold scuffing the wall in preparation of applying stucco on or about January 19, 2018, and times prior there to, the employer did not overlap the boards making the bridge to the other tower by placing them over the supports.  This exposes employee to falls from unsecured planks.

29 CFR 1926.451(b)(10): Scaffold components manufactured by different manufacturers were intermixed such that the components fit together with force and the scaffold's structural integrity was not maintained by the user    For the employees working on the scaffold scuffing the wall in preparation of applying stucco on or about January 19, 2018 and times prior there to the employer mixed scaffold frames requiring the decking to be placed over 2X6 lumber wedged there the side frame of the walk-through tubular welded frames.  This exposes employees to the hazard of falling.

This exposes employees to the hazard of falling from heights greater that 10 feet.

This exposes employees to falls from heights over 10 feet from the scaffold.

29 CFR 1926.454(b): The employer did not have each employee involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question.    For the employees working from the scaffold on or about January 19, 2018 and times prior there to, the employer did not train employees erecting scaffolding to recognize the hazards with the scaffold and how to properly erect scaffolding by a competent person.  This exposes employees to the hazard of falling from unguarded or not fully planked scaffold, or scaffold collapse.
