29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:  On or about January 19, 2018, employees were applying stucco to the commercial project under construction up to approximately 16 feet high from a tubular welded scaffold that did not have guard rails on the ends of platforms, exposing employees to a fall hazard.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  On or about January 19, 2018, employees were working from the tubular welded scaffolding to apply stucco to the commercial project under construction without being trained to recognize the hazards of working from scaffolding, exposing employees to a fall hazard.  Note: The training shall include the following areas, as applicable: a)  Any electrical hazards, fall hazards and falling object hazards in the work area; b)  The correct procedures for dealing with electrical hazards and for erecting, maintaining, and disassembling the fall protection systems and falling object protection systems being used; c)  The proper use of the scaffold, and the proper handling of materials on the scaffold; d)  The maximum intended load and load-carrying capacities of the scaffolds used; and e)  Any other pertinent requirements of this subpart.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access:  On or about January 19, 2018, employees were applying stucco to the commercial project under construction up to approximately 16 feet high from a tubular welded scaffold that included a portable extension ladder that did not extend at least 3 feet above the upper landing surface, exposing employees to a fall hazard.

Employees were using an extension ladder that was not placed one-quarter of the working length of the ladder, exposing employees to a fall hazard.
