Worksite: The Employer did not ensure that the circuit on which an employee was working was de-energized and rendered inoperative by the use of locks and or tags at points where such a circuit could have been energized. On or about 01/11/2018, the employee came in contact with energized electrical parts while working on the circuit and sustained an electrical shock and burn from the incident.

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided. Worksite: The Employer did not ensure that the employee who was testing a 277-volt electrical circuit was provided with and required to wear the appropriate personal protective equipment (PPE) to protect from electrical hazards, such as (but not limited to) electrical shock, arc flash/arc blast and electrocution. On or about 01/11/2018, the employee came in contact with energized electrical parts while working on the electrical circuit and sustained an electrical shock and burn.
