29 CFR 1910.28(b)(11)(ii): Each flight of stairs having at least 3 treads and at least 4 risers was not equipped with stair rail systems and handrails as required in Table D-2 of this standard: a) In the maintenance shop, near the parts desk, a flight of stairs leading to the second floor where parts are stored was not equipped with a stair rail on one open side, exposing employees to falls of 4 feet or greater while accessing the second floor platform. b) In the maintenance shop, near the parts desk, a flight of stairs leading to the second floor where parts are stored was missing a portion of its stair rail on one open side at the point where the stairs meet the platform, exposing employees to falls of 10'.
