The Impulsa 6000 automatic press weld machines, AW1 and AW2, located in the assembly weld area were not equipped with effective guarding such as, but not limited to, electronic eye safety circuits, two hand controls, or protection similar to that prescribed for punch press operations. Employees were exposed to caught in and amputation hazards.
