Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  (a): An employee suffered a partial amputation to his right index finger on 01/20/2018 at approximately 8:30 AM. OSHA was notified via online submission approximately 51.5 hours later on 01/22/2018 at 11:58 PM.   There is no abatement certification or documentation required for this item.
