29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    (a)  S&DC Corporation dba MBK Machine Corp located at 7450 Johnson Dr. Suite A Frederick, CO 80530: On or before January 23, 2018, the employer did not ensure that machine-specific procedures were developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in cleaning of and maintenance on the inside of CNC and milling machines.  Employees entering the inside of CNC and milling machines to facilitate maintenance and cleaning of scrap aluminum have not been provided adequate procedures to prevent inadvertent activation of the equipment. This exposed employees to the hazards of rotating and moving parts.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    (a)  S&DC Corporation dba MBK Machine Corp located at 7450 Johnson Dr. Suite A Frederick, CO 80530: On or before January 23, 2018, the employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed.  Employees performed maintenance on equipment subject to the lockout tag out standard, such as, but not limited to, CNC and milling machines.  The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure the procedure was followed.  This condition exposed employees to the hazards associated with rotating and moving parts.

This exposed employees to caught-in and struck-by hazards.    (b)  S&DC Corporation dba MBK Machine Corp located at 7450 Johnson Dr. Suite A Frederick, CO 80530: On or before January 23, 2018, the employer did not ensure that one or more methods of machine guarding was provided to protect the operator and other employees in the machine area of the Bridgeport Mill, Series I from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. This exposed employees to caught-in and struck-by hazards.    (c)  S&DC Corporation dba MBK Machine Corp located at 7450 Johnson Dr. Suite A Frederick, CO 80530:  On or before January 23, 2018, the employer did not ensure that one or more methods of machine guarding was provided to protect the operator and other employees in the machine area of the milling machines, including but not limited to the horizontal machining centers, vertical machining centers, and the CNC vertical machines, from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. This exposed employees to caught-in and struck-by hazards.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):    (a)  S&DC Corporation dba MBK Machine Corp located at 7450 Johnson Dr. Suite A Frederick, CO 80530:  On or before May 1, 2018, the employer did not ensure that energized electrical wires leading to the Clausing Colchester Lathe were contained in a cabinet or enclosure that meets the requirements of this standard. This condition exposed employees to electric shock hazards.
