29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use: (a) Aolanis Construction LLC located at 3502 Big Ben Rd. Fort Collins, CO 80526: On and before January 19, 2018, the exposing employer did not ensure that a rough terrain forklift was designed by the manufacturer to support a scaffold platform. A wooden job-made platform was connected to a Genie rough terrain forklift model GTH-844, and was being used to raise an employee to conduct framing work. The manufacturer's directions provide that this rough terrain forklift was not designed as a personnel lifting device. This condition exposed the employee to a fall hazard of up to approximately 23 feet.

29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: a) Aolanis Construction LLC located at 3502 Big Ben Rd. Fort Collins, CO 80526: On and before January 19, 2018, the exposing employer did not ensure that the entire scaffold platform was attached to the forks and that the forklift was not moved horizontally while the platform was occupied. A wooden job-made platform was connected to a Genie rough terrain forklift model GTH-844, and was being used to raise an employee to conduct framing work. The platform was not tied or secured to the forklift, and the employee was not using a personal fall arrest system. This condition exposed the employee to a fall hazard of up to approximately 23 feet.

Fort Collins, CO 80526: On and before January 19, 2018, the employer did not ensure that employees engaged in residential construction activities 6 feet or more above lower levels were protected by guardrail systems, safety net system, or personal fall arrest system. Employees were framing and sheathing the roof without the use of fall protection. This condition exposed employees to fall hazards of up to approximately 23 feet.
