29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    Location:  a) On the ground level on the Northwest side of the jobsite:    On or about 01/16/18, employees accessed the ground level from the basement level by going through an opening between guardrails.  The access point to eth ladder did not have a moveable guardrail and the employees did not use a fall arrest system

Location:  a) On the basement level on the Northwest side of the jobsite:    On or about 12/14/16, employer did not provide training to each employee who used a job-made ladder for access to the ground level that were exposed to a fall hazard to enable each employee to recognize the hazards related to ladders and the procedure to be followed in order to minimize these hazards.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed; the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.     Location: a) On the basement level on the Northwest side of the jobsite:    On or about 01/16/17, employees accessed the ground level from the basement level by using a job-made ladder. The ladder length did not allow the ladder side rails to extend at least 3 feet above the upper landing surface being accessed.
