29 CFR 1910.266(h)(2)(ii): Before each tree is felled, conditions such as, but not limited to, snow and ice accumulation, the wind, the lean of tree, dead limbs, and the location of other trees, shall be evaluated by the feller and precautions taken so a hazard is not created for an employee. a)An employee was exposed to hazards associated with felling trees in that no evaluation was conducted to protect employees from dead or falling limbs.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. a) On August 25, 2017, an employee was hospitalized for a work related injury and the employer did not report a reportable injury to OSHA within the required twenty-four hours.
