29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: On or about 01/10/2018 employer did not establish an energy control procedure and training for employees to de-energize equipment when cleaning the equipment at the end of shift in the saw area. Employees were exposed to amputation hazards on the Baldor Reliance Super E blower motor used to collect aluminum shavings on the number 9 straight cut saw.

29 CFR 1910.303(b)(1)(ii): Electrical equipment was not free from recognized hazards, based on the mechanical strength and durability, including, for parts designed to enclose and protect other equipment, the adequacy of the protection thus provided: Employees working near the stone saw in the saw department were exposed to electrical shock hazards in that the conduit containing the electrical supply to the east saw motor was broken and not maintained.
