29 CFR 1910.176(b): Storage of material created a hazard: a) Front storage yard: A steel walk way, weighing in excess of 10,000 pounds, was not securely stored on top of dunnage. The material fell onto an employee's foot after it was struck by a forklift resulting in an in-patient hospitalization, on or about 3/2/18. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.178(m)(1): Trucks were driven up to employees standing in front of benches or other fixed objects: a) Front storage yard: A forklift operator drove up to a stored walkway to load palletized material and two employees were working on the opposite side of the walkway loading additional materials. The forklift hit the walkway and knocked it off of the dunnage and onto one of the employee's feet resulting in an in-patient hospitalization, on or about 3/2/18. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
