29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) 7 RYCROFT ROAD, MECHANICSBURG, PA, ROOFTOP - The employer did not ensure that employees who were installing shingles on the 9/12 pitch roof were protected by fall protection, which thereby exposed the employees to fall hazards that ranged from 12 to 33 feet to the ground below, on or about January 24, 2018. No abatement certification or documentation required.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) 7 RYCROFT ROAD, MECHANICSBURG, PA, MAIN ROOFTOP - The employer did not ensure that the rails of the 32 foot Werner extension ladder that was being used to access the roof were at least three feet above the edge of the roof, which thereby exposed employees to fall hazards of approximately 21 feet, on or about January 24, 2018. (a) 7 RYCROFT ROAD, MECHANICSBURG, PA, GARAGE ROOFTOP - The employer did not ensure that the rails of the 20 foot Werner extension ladder that was being used to access the roof were at least three feet above the edge of the roof, which thereby exposed employees to fall hazards of approximately 12 feet, on or about January 24, 2018. No abatement certification or documentation required.
