29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: At Fryer Area: On or about January 24, 2018, and times prior thereto, employees were exposed to burn hazards while removing or adding hot oil/grease into the fryer machine to fry tortilla chips. The employees were not provided adequate personal protective equipment such as, but not limited, to aprons and gloves.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Corn Washing Area: On or about January 24, 2018, and times prior thereto, employees were exposed to caught-in hazards from a rotating chain when operating the Corn Washer Machine. b) Top section of the Corn Washer Machine: On or about January 24, 2018, and times prior thereto, employees were exposed to caught-in hazards from an unguarded rotating chain while processing the washed corn. c) Tortilla Cooking Area: On or about January 24, 2018, and at times prior thereto, employees were exposed to caught-in hazards from rotating parts in the Tortilla Maker Machine which had a missing panel cover while cooking tortillas. d) Operator Station Area: On or about January 24, 2018, and at times prior thereto, employees were exposed to caught-in hazards from rollers rotating in the Tortilla Maker Machine while inserting dough for the tortilla cooking process.

29 CFR 1910.305(j)(2)(iv): A receptacle installed in a wet or damp location was not effectively enclosed for the location. Corn Washer Machine Area: On or about January 24, 2018, and at times prior thereto, employees were exposed to electrical hazards while using a wet process to wash corn. The toggle switch used to energize the corn washer machine was missing the water proof cover.
