Location: a) On the running platform/car sling for elevator shaft B: On or about 01/23/18, employees were working on a running platform/car sling that did not have a temporary three button run control that take activation of two buttons to run any direction and did not have an emergency stop switch on a stand-alone circuit. The employees are not protected from control malfunction which cause the employees exposed to crush by and/or struck by hazard.

Location: a) On the running platform/car sling for elevator shaft B prior to Site Safety Manager warning: On or about 01/23/18, employees were exposed to falling up to approximately 67 feet from the running platform/car sling down to the bottom of the elevator shaft pit. The employees were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. b) On the running platform/car sling for elevator shaft B after to Site Safety Manager warning: On or about 01/23/18, an employee were exposed to falling up to approximately 94 feet from the running platform/car sling down to the bottom of the elevator shaft pit. The employee was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.
