29 CFR 1910.36(a)(3): An opening into an exit must be protected by a self-closing fire door that remains closed or automatically closes in an emergency upon the sounding of a fire alarm or employee alarm system: a) Building 58, where openings into exits not protected by self-closing fire doors included: 1. Second Floor, where the two fire doors to the stairwell located on the northwest side of the building were propped opened. 2. Second Floor, where the stairwells designated as emergency exits were not equipped with self-closing fire doors. 3. Mezzanine, where the stairwells designated as emergency exits were not equipped with self-closing fire doors.

29 CFR 1910.36(d)(1): Employees must be able to open an exit route door from the inside at all times without keys, tools, or special knowledge. A device such as a panic bar that locks only from the outside is permitted on exit discharge doors. a) Building 58, where designated emergency exits requiring special knowledge for operation included: 1. First Floor, where the metal roll-up door had to be opened manually by pulling on the chain that engaged the pulley. 2. First Floor, where the gate located on the exterior side of the roll-up door required a key to unlock the padlock that was used to secure the gate.
