(A) On or about Jan. 18, 2018, located on the west side of a commercial structure, an employee was exposed to fall hazards while performing work on a second story floor structure near a 55 inch by 49 inch floor hole that did not have a proper cover of suitable strength in place. Potential fall heights were measured to be greater than 14 feet above the ground below.    (B) On Jan. 19, 2018, located on the west side of a commercial structure, an employee of De Am-Ron Building Systems, LLC, a subcontractor to Stonehenge Building Group, Ltd, was exposed to fall hazards while performing grouting work on a second story floor structure near a 55 inch by 49 inch floor hole that did not have a proper cover of suitable strength in place. Potential fall heights were measured to be greater than 14 feet above the ground below.
