29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Patty packaging line #1: On or about January 13, 2018, and at times prior, the employer exposed employees to caught-in, and amputation hazards, in that the metal detector portion of the patty packing conveyor was not guarded in a way that prevented employees from reaching between the moving conveyor belt(s) to the inside of the machine. b) Patty packaging line #2: On or about January 13, 2018, and at times prior, the employer exposed employees to caught-in, and amputation hazards, in that the metal detector portion of the patty packing conveyor was not guarded in a way that prevented employees from reaching between the moving conveyor belt(s) to the inside of the machine. c) Patty packaging line #3: On or about January 13, 2018, and at times prior, the employer exposed employees to caught-in, and amputation hazards, in that the metal detector portion of the patty packing conveyor was not guarded in a way that prevented employees from reaching between the moving conveyor belt(s) to the inside of the machine. d) Patty packaging line #4: On or about January 13, 2018, and at times prior, the employer exposed employees to caught-in, and amputation hazards, in that the metal detector portion of the patty packing conveyor was not guarded in a way that prevented employees from reaching between the moving conveyor belt(s) to the inside of the machine. e) Patty packaging line #5: On or about January 13, 2018, and at times prior, the employer exposed employees to caught-in, and amputation hazards, in that the metal detector portion of the patty packing conveyor was not guarded in a way that prevented employees from reaching between the moving conveyor belt(s) to the inside of the machine. f) Patty packaging line #6: On or about January 13, 2018, and at times prior, the employer exposed employees to caught-in, and amputation hazards, in that the metal detector portion of the patty packing conveyor was not guarded in a way that prevented employees from reaching between the moving conveyor belt(s) to the inside of the machine.
