29 CFR 1910.22(a)(3): Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice: (a) Grand Junction SSC FAA, at Douglas Pass, Grand Junction, CO 81506: On and before January 24, 2018, the Agency did not ensure that walking-working surfaces were maintained free of hazards such as protruding objects and other trip hazards. The Agency allowed storage of various material on the walkway between the beacon arrays, creating trip hazards, and minimizing the area employees could safely walk in. This condition exposed employees to trippingand fall hazards.

29 CFR 1910.147(c)(5)(ii)(C)(1): Lockout devices were not substantial enough to prevent removal without the use of excessive force or unusual techniques, such as with the use of bolt cutters or other metal cutting tools: (a) Grand Junction SSC FAA, at Douglas Pass, Grand Junction, CO 81506: On and before January 24, 2018, the Agency did not ensure that an energy isolating device was substantial enough to prevent removal without the use of excessive force or unusual techniques, such as with the use of bolt cutters or other metal cutting tools. The lockout device used to secure the sail on the long range radar at the Radome near Douglas pass was not substantial enough to prevent the dome from rotating when employees were attempting to climb the radar panel during maintenance. This condition exposed employees to fall hazards.
