29 CFR 1910.28(b)(1)(i):  The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following:  guardrail system, safety net system, or personal fall protection system:    a)  Upper storage level, south side:  On or about January 24, 2018, and at times prior thereto, employees were exposed to a fall hazard of greater than 4 feet, while storing equipment and supplies on an elevated platform without a toprail and midrail.    b)  Upper storage level, west side:  On or about January 24, 2018, and at times prior thereto, employees were exposed to a fall hazard of greater than 4 feet, while storing equipment and supplies on an elevated platform without a toprail and midrail.

29 CFR 1910.213(c)(1): Each circular hand-fed ripsaw was not guarded by a hood which completely enclosed that portion of the saw above the table and that portion of the saw above the material being cut:     Near the south side of the shop:  On or about January 24, 2018, and at times prior thereto, employees were exposed to an amputation hazard, while operating the Grizzly Table Saw Model G0690 without the blade guard in place.

29 CFR 1910.213(c)(2): Each hand-fed circular ripsaw was not furnished with a spreader to prevent material from squeezing the saw or being thrown back on the operator. The spreader was not made of hard tempered steel, or its equivalent, and was not thinner than the saw kerf. It was not of sufficient width to provide adequate stiffness or rigidity to resist any reasonable side thrust or blow tending to bend or throw it out of position. The spreader was not attached so that it will remain in true alignment with the saw even when either the saw or table is tilted:  On or about January 24, 2018, and at times prior thereto, employees were exposed to a struck-by hazard, while operating the Grizzly 10" Table Saw, Model G0690 without a spreader attached.

29 CFR 1910.213(p)(4): Belt sanding machines were not provided with guards at each nip point where the sanding belt runs on to a pulley. These guards would effectively prevent the hands or fingers of the operator from coming in contact with the nip points. The unused run of the sanding belt was not guarded against accidental contact:    Near the south side of the shop:  On or about January 24, 2018, an employee was exposed to an amputation hazard, when using the Ritter horizontal belt sander, Model R905, without guards.
