29 CFR 1910.1200(f)(6)(ii): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals. a) Warehouse area- On or about 1/23/2018, and at the times prior, the employer exposed employees to fire hazards in that the employer failed to provide labels regarding the hazards of the chemicals for propane cylinders that were utilized for forklift trucks when employees operating forklift.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: a) Throughout facility - On or about 1/23/2018, and at the times prior, the employer exposed employees to skin burns and fire hazards in that the employer failed to provide information and training specific to the hazards of chemicals used at the workplace when employees operating forklift equipped with propane gas (highly flammable) and working with Ultra Floor Cleaner (corrosive) and Auto-Chlor Solution-QA Sanitizing products. Repeat - Other-than-Serious : Five Star Food Service was previously cited for Other-than-Serious violation of the same occupational safety and health standard and hazard of 29 CFR 1910.1200(h)(1), which was contained in OSHA inspection # 1243657, Citation 2, Item 4 and was affirmed as final order on 11/06/2017, with respect to a workplace located at 1853 W. North Main Street, La Fayette, GA 30728.
